WHO IS ELIGIBLE?

  • Those who currently have traditional Medicare insurance benefits
  • A patient who has been referred by a physician to home healthcare for skilled services (e.g. nursing, therapy)
  • A patient who is considered “homebound” by Medicare guidelines, meaning it is difficult for the patient to leave the home

WHAT SERVICES ARE PROVIDED?

Our Nurses make home visits to patients to evaluate and monitor their condition. Each nurse collaborates with the patient, caregiver, family and care team, under the direction of the physician, to assure the individualized collaborative treatment plan is carried out. Nurses assess and teach patients about medications, disease process, diet, vital signs and any pertinent nursing procedures (e.g. injections and wound care). Our goal is to help patients and their caregivers achieve independence and thrive safely in the home.

Skilled Nursing services include:

  • Wound care
  • Medication management
  • IV therapy
  • Pain management
  • Anti-coagulation therapy & PT/INR testing
  • Thorough education on disease state

Physical Therapists (PT) are movement experts who help patients manage their illness or condition, restore function and optimize quality of life through functional exercise, hands on treatment and patient education. PT’s assess each patient and develop an individualized collaborative treatment plan, under the direction of the physician, together with the patient, caregiver, family and care team.

Physical Therapy services include:

  • Fall prevention and home safety assessment
  • Functional exercise programs to promote strength, balance, endurance and mobility
  • Gait (walking) and transfer training and education
  • Instruction on pain management and relief
  • Lymphedema Therapy
  • Durable medical equipment recommendations and adjustments

Occupational Therapists (OT) assist patients in developing, recovering or modifying the meaningful activities, or occupations, of daily living (e.g. bathing, dressing). OT’s assess patients after an injury or diagnosis and, under the direction of the physician, develop an individualized collaborative treatment plan together with the patient, caregiver, family and care team.

Occupational Therapy services include:

  • Customized treatment programs to improve one’s ability to perform daily activities
  • Functional exercise programs for strength, balance and increased proprioception
  • Comprehensive home assessment
  • Adaptive equipment and durable medical equipment recommendations, usage and training
  • Wheelchair assessment, positioning and fitting
  • Instruction on proper use of prosthetics and orthotics

Speech Language Pathologists (SLP) assist patients who have swallowing or communication difficulties as a result of an illness or medical condition. SLP’s assess each patient and develop an individualized collaborative treatment plan, under the direction of the physician, together with the patient, caregiver, family and care team.

Speech Therapy services include:

  • Evaluation and treatment in the areas of speech, language, swallowing, cognition, memory and safety
  • Guidance for diet modification
  • Interventions to improve problem solving, orientation skills, and recall
  • Communication boards and devices
  • Voice therapy

Medical Social Workers (MSW) assist patients and families in understanding the social and emotional factors related to the patient’s illness or medical condition. By providing emotional and mental assessments, MSW’s help patients develop coping strategies and take appropriate action to obtain available community resources and support. MSW’s, under the direction of the physician, develop an individualized collaborative treatment plan together with the patient, caregiver, family and care team.

Home Health Aides assist patients with their Activities of Daily Living (ADL’s), such as bathing and grooming. They work as part of the multidisciplinary team to assure the patient is progressing with their goals.
