a variety of disciplines, including nursing, therapy, rehabilitation or any combination thereof. Our professional staff is highly skilled and trained to treat patients with all types of medical conditions.

PulmoCare is the flagship specialty program at Activa. Respiratory therapists, nurses and physical therapists work together to provide the most comprehensive respiratory care and pulmonary rehabilitation available in the home setting. Spirometry, oximetry, peak flow, and incentive spirometers are all used as part of the care plan.

CardioCare takes a multi-disciplinary approach to managing CHF and other cardiac related conditions. The program ensures the delivery of evidence-based best clinical practice standards and promotes positive outcomes to each individual patient.

OrthoCare specializes in individualized, home based physical therapy for patients who have had bone or joint replacement surgery. This program is managed by physical therapists with extensive experience and clinical expertise with joint replacement.

DiabetiCare is truly a one-of-a-kind program. First, patients are taught how to manage the disease through diet, exercise and blood glucose monitoring. As with all agencies, once a patient can demonstrate the ability to self-manage their diabetes they are likely to be discharged from the Agency’s direct care. However, unlike other agencies, Activa continues to monitor their patients through the use of a telemonitoring system that stays with the patient after discharge from service. When changes in the patient’s condition occur, early intervention is possible and hospital readmissions are reduced.

NeuroCare specializes in treating patients who have had a neurological injury or condition such as a stroke or progressive neurological disease. Activa’s team of nurses, physical, occupational and speech therapists provides comprehensive rehabilitation and medical management with the goal of improving functional ability and fostering patient independence.
