For hourly Home Healthcare, our Registered Nurse will conduct an initial assessment to determine the type of home healthcare services that best meet your needs and tastes. If you are a Medicare beneficiary, our clinical team will consult with your physician to determine the type of skilled services you will need for your specific medical condition.

Where can I receive home health services?

Medicare services must be provided in your residence, which can be your home, an Assisted Living Facility or an Independent Living Facility.

Private duty services can be provided anywhere you need them.

What will it cost to have home health services?

Services provided under the Medicare benefit are covered at 100%. Prices for private duty services, such as hourly or live-in care, vary depending on the level of service you select. Call 561-819-4112 to ask about our monthly specials.

Can you assist me if I speak another language other than English?

Activa has some bilingual staff that are fluent in more than one language. If yours is not a language that they are proficient in, they have the ability to use an interpretation service to assist with communication. For more information, click on the following link: Click Here

What are your hours of operation?

A member of our professional staff is available to assist you 24 hours a day, 7 days a week.

How are your Caregivers screened?

We interview applicants for compassion and tenderness to ensure you will feel comfortable with your caregiver. Additionally, our caregivers must meet the following strict standards:

  • Criminal background check
  • Verified work history and reference checks
  • State licensure appropriate to caregiver skill level
  • Professional liability insurance – minimum limits of $1,000,000/500,000
  • Mandatory education, training and skills assessment

What if I don’t like my Caregiver?

Our proprietary Caregiver matching process generally does a great job at finding the most suitable Caregiver for your personal needs and taste. If for any reason, at any time, you are not satisfied with your Caregiver, we will find you a replacement and credit you for 24 hours of care. That’s our Caregiver Guarantee and our promise to you!

What is your cancellation policy for private duty services?

There are no long term contracts to sign. You may change the level of services or cancel your services all together at any time and at no additional cost.

How do I get started?

If you think that you may need skilled services offered under your Medicare benefit, you can ask your physician to make a referral to Activa Home Health. You can also call us at 561-819-4112 and we will assist you by contacting your physician.

If you are interested in private duty services, we have a simple 4-step process:

  1. Contact us by using the form to the right or by calling 561-819-4112. We will give you an overview of the services we offer and will answer all of your questions.
  2. A Registered Nurse from Activa will conduct a FREE in-home assessment and discuss a plan of care that will match your needs and desires.
  3. Caregiver Match: Once we understand your needs and personal preferences, our staff will complete an extensive search of our caregiver profiles to make the best match for you. Our Caregiver Guarantee means that if at any time you are not satisfied with your Caregiver we will find a replacement and credit you for 24 hours of service!
  4. Start of Service: After the service agreement has been reviewed and signed, service can immediately begin. There is no long term obligation for service.
